I Who's Most at Risk?

Infants under 6 months
Their small airways make RSV especially dangerous. Premature babies (born before 35 weeks) and those with heart or lung conditions are at highest risk for severe illness or hospitalization.

Babies 6–12 months
Still at elevated risk during their first RSV season. Most hospitalizations occur in infants under 12 months. RSV is the leading cause of infant hospitalization in the US.

Adults 60 and older
RSV can cause severe lower respiratory illness in older adults, comparable to flu. An estimated 60,000–160,000 older adults are hospitalized for RSV each year in the US.

Immunocompromised & chronic conditions
People with COPD, asthma, heart failure, or weakened immune systems face a higher risk of RSV progressing to pneumonia or bronchiolitis.
RSV is the #1 cause of hospitalization for US infants. Most children have had RSV by age 2 — reinfection is common because immunity is short-lived.
II The Protection Window
For newborns & infants
Nirsevimab (Beyfortus)
A monoclonal antibody — not a vaccine — that gives infants immediate, passive protection against RSV. Recommended for all babies under 8 months entering their first RSV season, and some high-risk children 8–19 months. A single injection, typically given before the season starts or shortly after birth during RSV season.
Given once, before RSV season
For pregnant people
Maternal Abrysvo (RSV vaccine in pregnancy)
Pfizer's Abrysvo vaccine can be given at 32–36 weeks of pregnancy. Maternal antibodies cross the placenta and protect the newborn for the first few months of life — the window of highest vulnerability. Given as an alternative or complement to Beyfortus depending on your timing and plan.
32–36 weeks gestation
For adults 60+
Abrysvo (Pfizer) or mRESVIA (Moderna)
FDA-approved RSV vaccines for adults 60 and older. Both provide strong protection against RSV lower respiratory disease. Recommended as a one-time vaccination; timing guidelines are still evolving — talk to your doctor or pharmacist.
One-time dose, ideally before fall
Talk to your pediatrician at the next well-child visit. Beyfortus availability varies by practice and season. Ask before October — supplies can run low. If you're pregnant, ask your OB about Abrysvo at your 32-week appointment. Not medical advice.
III Symptoms & When to Act
RSV starts like a cold — runny nose, mild cough, low-grade fever. Most children recover at home in 1–2 weeks. The concern is when it moves to the lower airways, causing bronchiolitis or pneumonia.

Runny nose, mild cough, low-grade fever (under 100.4°F in infants under 3 months: call your doctor regardless). Eating and drinking normally. Staying alert and engaged.

Call your pediatrician today
Fever above 100.4°F in an infant under 3 months. Poor feeding — eating less than half of normal. Cough getting worse after day 3. Fewer wet diapers than usual. Wheezing on exhale.

Emergency — go now or call 911
Fast, labored breathing. Nostrils flaring or ribs showing with each breath (retractions). Blue or gray color around lips, fingernails, or skin. SpO₂ below 95% on a pulse oximeter. No wet diapers for 6+ hours. Unresponsive or limp. Struggling to breathe at all.
